Distal tubal obstruction affects the end of the fallopian tube nearest the ovary, where the fimbriae normally catch the released egg. It is commonly caused by pelvic infection or pelvic inflammatory disease and often leads to fluid collecting in the tube (hydrosalpinx). Because the fimbrial lining is often damaged even if surgery restores an opening, IVF is frequently recommended over tubal surgery once distal disease is significant, particularly when a hydrosalpinx is present.
