An endometrioma is a cyst on the ovary formed when endometriosis tissue bleeds and accumulates within the ovary, producing thick, dark fluid, which is why it is sometimes called a chocolate cyst. It is a marker of endometriosis and can affect ovarian reserve, either from the cyst itself or from surgery to remove it. Management depends on cyst size, symptoms, and fertility plans, and is decided with a gynaecologist, balancing the benefits of surgery against the risk of reducing egg reserve further.
