A freeze-all cycle is an IVF cycle in which all embryos created are frozen (vitrified) for later use, rather than transferring one embryo fresh in the same cycle. Reasons for choosing this approach include a high risk of OHSS after stimulation, an unfavourable endometrial lining at the time embryos are ready, a need for genetic testing of embryos (PGT) before transfer, or specific stimulation protocols such as progestin-primed ovarian stimulation. Evidence has shown that, for some patients, transferring a frozen embryo in a later, unstimulated or hormonally prepared cycle can give the endometrium a better chance to be receptive than a fresh transfer following stimulation. The decision to freeze all embryos is made by the treating fertility team based on the individual cycle, not as a routine default for everyone.
