Ovarian drilling is a laparoscopic surgical procedure in which small punctures are made in the surface of the ovary using heat or a laser, mainly used to help restore ovulation in women with polycystic ovary syndrome (PCOS) who have not responded to ovulation-inducing medicines. It is generally considered a second-line option, used after medicines such as letrozole or clomiphene, or gonadotropins, have not led to ovulation or pregnancy. Because it is a surgical procedure, it carries the usual risks of laparoscopy and can, in some cases, reduce ovarian reserve, so it is used selectively and discussed with a specialist.
