Sex hormone-binding globulin carries testosterone and oestrogen in the bloodstream, controlling how much is freely active. Low SHBG, often seen with insulin resistance, raises free testosterone and is one reason androgen symptoms can appear in PCOS even when total testosterone looks normal.
