Ovarian cystectomy is a surgical procedure, most often performed laparoscopically, to remove an ovarian cyst such as a dermoid cyst or endometrioma while trying to preserve as much healthy ovarian tissue as possible. It is usually recommended when a cyst is large, persistent, causing symptoms, or when there is uncertainty about whether it could be something other than a benign cyst. Because the surgery involves removing tissue from the ovary, it can reduce ovarian reserve to some degree, particularly with endometriomas, so the decision to operate balances the benefits of removing the cyst against this risk, especially in women trying to conceive.
