Ovarian transposition (oophoropexy) is a surgical procedure that repositions one or both ovaries away from the area planned for pelvic radiotherapy, aiming to reduce radiation exposure and protect ovarian function. It is used selectively, usually alongside other fertility preservation options such as egg or embryo freezing, and does not remove all risk of reduced ovarian function after treatment. Decisions about ovarian transposition are made jointly by the oncology, radiation oncology and fertility teams, taking into account the radiotherapy field, urgency of cancer treatment and the person's fertility goals.
