Pyosalpinx is a fallopian tube distended with pus, seen during acute pelvic infection such as severe pelvic inflammatory disease. It is a marker of active infection needing prompt medical assessment and treatment, and can progress to a tubo-ovarian abscess if untreated. After the acute infection resolves, the affected tube is often left scarred or blocked, which is one of the ways pelvic infection contributes to tubal-factor infertility later on.
