Random-start stimulation is an ovarian stimulation protocol that can begin on any day of the menstrual cycle rather than waiting for the next period, allowing egg or embryo freezing to proceed more quickly. This is particularly useful when fertility preservation is needed urgently before cancer treatment, since it can shorten the time between referral and egg retrieval by one to several weeks compared with waiting for a conventional cycle start. The number of mature eggs collected and their suitability for freezing are broadly comparable to conventional-start protocols in most reported experience, though the choice of protocol is individualised by the fertility specialist based on hormone levels, scan findings and how urgently treatment is needed.
