Adverse event reporting

Clinics are required to report serious incidents and to be open with patients when something goes wrong. Recent Australian embryo and sample errors prompted the national review of regulation. If you are concerned about your care, you can raise it with the clinic, your state health complaints commissioner, or Ahpra.
