High DNA fragmentation has been associated with lower fertilisation and higher miscarriage rates in some studies, but there is no agreed threshold and evidence that changing treatment on the basis of the result improves live birth is limited. Canadian practice generally treats it as an adjunct in selected cases, such as recurrent loss or repeated failed cycles, rather than a routine test.
