Hypothalamic amenorrhoea is the absence of periods caused by the hypothalamus reducing its hormone signals to the ovaries, rather than a problem within the ovaries themselves. It is often related to low body weight, rapid weight loss, very intense exercise, chronic stress, or a combination of these, and results in low oestrogen and infrequent or absent ovulation. Management usually focuses on addressing the underlying trigger, such as restoring adequate nutrition, moderating exercise load, and reducing stress, often with support from a dietitian or counsellor alongside a fertility clinician. Ovulation often returns once the underlying cause is addressed, though some women may also need ovulation induction.
