GnRH agonists initially cause a brief surge of FSH and LH before switching the pituitary off, which prevents an early natural LH surge. They are used for down-regulation in long protocols, sometimes as a trigger, and in other gynaecological conditions. Temporary menopause-like effects such as hot flushes can occur while the pituitary is suppressed.
